READ MORE Second dose reminder for COVID-19 vaccinationsIf you received your first COVID-19 vaccination on April 1, your second dose is scheduled for Thursday, April 29. Your appointment time is on the back of your vaccination record card.
Please remember to bring your vaccination record card, complete this form again and print it to bring with you to your appointment. The clinic will be held at the Wellness Center on West Campus, at the same location as the first clinic.
