Outside Activities Approval Form
As the academic year gets underway, The Office of Human Resources would like to remind employees of the Approval Request for Outside Activities form available via my.gcsu.edu. The purpose of this form is for employees to disclose any potential, actual or apparent conflicts of interest before engaging in compensated outside activities that relate to their expertise or responsibilities as a USG employee. Such activities may include, but are not limited to, consulting, teaching, speaking, offering services, etc. The new form is effective immediately. Please refer to corresponding USG policy information (cited at the top of the form).
After logging into my.gcsu.edu, you will locate Forms from the Service Apps menu, then scroll down and locate University Required Forms.
